The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

M.S.H.S.L.. Parent Permit and Health

Questionnaire / Athletic Eligibility Statement.

_____ 2. An up-to-date SPORTS physical exam within the

last three years.

_____ 3. Pay participation fee (Make your check payable

to Central H.S. ) ($150.00)

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..

Season Starts: April 4, 2016

Practices: Monday - Friday @ McMurray Fields or Central High School, typically 5:30 to 7:00 p.m.
