Lacrosse Information Meeting will be held Wednesday, February 25th at Central High School in the Cafeteria at 7:00 p.m.

The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

_____ 1. A completed & signed 2014-2015

M.S.H.S.L.. Parent Permit and Health

Questionnaire / Athletic Eligibility Statement.

_____ 2. An up-to-date SPORTS physical exam within the

last three years.

_____ 3. Pay participation fee (Make your check payable

to Central H.S. )

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..
