BASEBALL

Season starts: 3/17/14. Meeting Time & Place: M-F 2:30 - 5:00 pm at Dunning baseball fields.  Description:  Competitive at 3 levels (Varsity, J.V. & 9th grade). Participation Fee: $45.00 /

with fee waiver: $20.00

 

The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

_____ 1.  A completed & signed 2013-2014

                 M.S.H.S.L.. Parent Permit and Health

                 Questionnaire / Athletic Eligibility Statement.

_____ 2.   An up-to-date SPORTS physical exam within the

                 last three years on file with School Nurse. 

_____ 3.   Pay participation fee (Make your check payable

                 to Central H.S. )

 

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..
