Basketball Season starts on November 10, 2014.

 

The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

_____ 1.  A completed & signed 2014-2015

                 M.S.H.S.L.. Parent Permit and Health

                 Questionnaire / Athletic Eligibility Statement.

_____ 2.   An up-to-date SPORTS physical exam within the

                 last three years on file with School Nurse. 

_____ 3.   Pay participation fee (Make your check payable

                 to Central H.S. )

 

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..

 

*No participation fee refund will be given after the second official week of practice.
