Description:  Competitive at 3 levels, tryouts determine the team. Meeting Time & Place: Season starts: 8/12/13 M-F 9:0 –11:30am & 1:00 – 2:30pm, after 9/3 - 2:30 to 4:30pm

on soccer fields.

Participation Fee: $45.00 /with fee waiver: $20.00

The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

_____ 1.  A completed & signed 2013-2014

M.S.H.S.L.. Parent Permit and Health Questionnaire / Athletic Eligibility Statement.

_____ 2.   An up-to-date SPORTS physical exam within the last three years on file with School Nurse. 

_____ 3.   Pay participation fee (Make your check payable to Central H.S. )

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..

Do not mail forms and the participation fee to Central H.S., they must be hand delivered to the Athletic Office (room 1116) for verification PRIOR to or on the FRIDAY BEFORE the first scheduled day of practice.
