Student/Athlete Checklist

1) Athletic Office Forms

2) Up to date physical

*3) Activity Fee $45 or $20 for free and reduced lunch

*4) Uniform Fee to be determined, Usually $40-$50(everybody must pay)

5) Racquet (see other players or the coaches if you are not sure what type to purchase)

6) Proper footwear

*Activity Fees and Uniform Fees can be paid by cash or check, please make checks out to JHS High School or JHS Athletics. Include on the check the name of the player in the memo blank. You must have all fees paid in order to participate in any match or practice. The cost of the uniform, which will be the players to keep at the end of the season, varies depending on the design we order. More information will follow.

Please, turn in all paperwork and payment in an envelope with your name, grade and CIF# to the Athletic Office
