The following check list of forms and participation fees are due FRIDAY (MARCH 27) 

_____ 1.  A completed & signed 2014-2015

                 M.S.H.S.L.. Parent Permit and Health

                 Questionnaire / Athletic Eligibility Statement.

_____ 2.   An up-to-date SPORTS physical exam within the

                 last three years. 

_____ 3.   Pay participation fee of $400 (minus the $100 deposit if you made one in November (Make your check payable  to Central H.S. )

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..OR YOU WILL NOT PRACTICE
