Join Golf, be on a team.

The following is a checklist in order to be eligible to participate on a team at Johnson High School.

¨ Sports Physical on file in the Athletic Office. Sports physicals are valid for three years.

¨ Minnesota State High School League Form completed and turned in to the Athletic Office. One time per school year.

¨ Participation Fee: $45.00 or $20.00 if you have free or reduced lunch. This Participation Fee should be turned into the Athletic Office.
